Amendments

Researchers wishing to make an amendment to an ethically approved research project are required to complete a Request for Amendment form

Researchers wishing to notify the Ethics Committee of a change in personnel to an existing approved research project are required to complete a Notification of Change in Personnel form.

Researchers are advised that if an amendment relates to the addition of a new site and a new Principal Investigator at the site, researchers are required to include the Notification of Change in Personnel form.
