Research amendments

Any changes to ethically approved research projects, including details of the personnel involved, must be approved by the ethics committees. Forms for notifying the committees of these changes can be found here.

Researchers wishing to make an amendment to an ethically approved research project are required to complete a Request for amendment form.

Researchers wishing to notify the Ethics Committee of a change in personnel to an existing approved research project are required to complete a Change in Personnel form.

Researchers are advised that if an amendment relates to the addition of a new site and a new Principal Investigator at the site, researchers are required to provide both forms.
