Palliative and end of life care with primary care

Primary health care provides continuity of care and information across the cancer continuum, including palliative and end-of-life care.

This is part of the normal scope of practice for providers in delivering person-centred care.

The provision of accessible, culturally appropriate and high-quality end-of-life cancer care to those affected by cancer is an important intersection between cancer services and primary health care.
